I understand that this group has a zero-tolerance policy for bullying or inappropriate behavior by either parents or children, and that any violation will result in termination of membership without a refund.*

I understand that parents and legal guardians are responsible for supervising their children at all times during activities and field trips.*

I understand that advance registration and payment are required. Field trips are limited to FTF members and their family members. All fees are nonrefundable unless Field Trip Friends Inc. cancels the event.

I understand that photos/videos of me and/or my family may be used on the Field Trip Friends website, social media, and shared with sponsors/partners to showcase our programs, field trips, and community impact.*

I understand FTF activities involve risks. I agree to release and hold harmless Field Trip Friends Inc. and its representatives from claims, injuries, damages, or losses arising from participation, to the fullest extent permitted by law.*

By signing this form, I certify that the information I have provided is accurate and complete, and I agree to the terms and conditions applicable to my participation and the participation of my family members in Field Trip Friends Inc. activities.*
